What to Bring
To ensure a smooth and efficient visit, please bring the following items with you:
Identification Card: A valid form of identification, such as a driver’s license, state ID, or passport, is required for check-in.
Insurance Card: If you plan to use insurance for your visit or materials, please have your insurance card ready.
Payment Method: Be prepared to cover co-pays and any services or materials not covered by insurance. We accept cash, checks, debit cards, Visa, and MasterCard.
Current Eyeglasses: Even if your glasses are old or broken, please bring them along.
Medication Information: A list of any medications you are currently taking, including names, dosages, and frequencies.
Contact Lens Prescription: If your visit involves contact lenses, bring your most recent contact lens prescription.
Transportation: If your eyes will be dilated during the visit, we recommend arranging for someone to drive you home.
Minors: Patients under the age of 18 should be accompanied by a parent or guardian.
